This form is being utilized to collect information regarding your forthcoming equipment return date so as to support organization of equipment for the 2020-21 season.  Please complete the form to the best of your ability. Equipment for your team must be returned by a single individual to Flames Community Arenas on your assigned date and time.  If you are collecting and returning equipment for multiple teams, you will need to complete and submit this form multiple times.
