Covid Self-isolation Tracker

In order to track the most up to date information for potential Covid exposures within our TBird family and to allow us to provide the safest environment within each team, skill group or clinic, we are asking for anyone who is entering self-isolation to let both their team HCSP and the association know.

This form is to be used only to identify TBird players or team staff. We expect that all families who have been exposed through other members of the household will follow all relevant isolation requirements.

In all cases it is important for the availability tracking on TeamSnap be kept up to date.

Have at least one of the listed Covid symptoms on the BCCDC website.

Close contact declaration for any one of the following:
a) They have been identified as a close contact either by health authorities
b) Someone they live with has had symptoms or received a positive Covid test and they are not fully vaccinated
c) They have had more than 15 minutes of close contact with someone where Covid protocols were not adhered to who has Covid symptoms or has received a positive Covid test
